Terms and Conditions Health and Medical The following are the terms and conditions for the PADI Courses. Anyone over the age of 15 and in good health can apply for this course. (Children over the age of 10 may apply for the Junior Open Water Scuba Diver License) Diving requires an acceptable level of health in certain areas. For example: heart problems, flu, sinus congestion, epilepsy, asthma, obesity, alcoholism, if you are taking any prescription medications, or if you have any other serious health problems. In addition, for those who could be pregnant or are trying to become pregnant, please refrain from diving. Please consult with your doctor and instructor before you decide to dive with us. Should any of the medical conditions listed on the PADI Medical Statements apply to you, please consult your doctor and have your PADI Medical Record and Certificate signed. If you are unable to do so, you will not be able to participate. You are required to sign the following documents: ● PADI Safe Diving Standards ● Guideline Understanding Statements ● PADI Negligence Agreements ● PADI Medical Record and Certificate When applying for a specific course you will be required to fill in an application form. Anyone under age 18 will need a parents signature on the application form. PADI Medical Record and Certificate form / pdf Rental Equipment When taking a beginner course, please do not bring your own equipment and please use East Dive’s rental equipment exclusively. The reason for this is East Dive cannot ensure the safety of equipment other than our own. Regarding wetsuit rental: We do not have rental gear for people larger than 190cm in height, 110kg in weight and 30cm foot size. Those wearing prescription glasses stronger than -0.5 are recommended to purchase prescription lenses for their mask. We have lenses available varying in strength from 1.0 to 8.0 in increments of 0.5. Please come to the dive shop or contact email prior to diving in order to select lenses for your mask. You can expect to receive the mask in 4 to 5 days or we bring it on the your training day. Regarding travel to higher altitudes after diving All scuba diving students must wait 24 hours after the dive if they are going to higher altitudes or traveling by plane after the dive. Please check your schedule and decide when to take the diving course. Diving Schedule All diving schedules are organized by East Dive. Participants can request specific days for specific courses. East Dive cannot refund the diving charge in the following situations: ● If the participant is not in good health ● If the participant does not arrive on time for the departure and does not contact East Dive ● If the paperwork is incomplete ● If sufficient knowledge and skill in diving has not been demonstrated ● If a traffic accident or weather conditions prevent access to the dive site Refunds cannot be given for cancellations made by the customer for any reason. Attendance Period All diving courses must be completed within 6 months. There will be an additional charge of ¥20,000 per day for courses which have been started more than six months prior. We recommend that participants keep their training days as close together as possible to ensure knowledge and skill retention. Cancellations dive course If during a course condition are unsuitable for diving or if conditions worsen between dives and a course in unable to be competed, students have two options for continuing their training: 1. A free rescheduled course with East Dive 2. A PADI transfer form to continue the course with another PADI dive shop 3. Refunds cannot be given for cancellations made by the customer for any reason. Cancellation certified diving If a cancellation is made, a fee will be charged depending on the number of days prior to the booking. ●Cancellations between from application date and before one month will cancellation is free ●Cancellations between one month and one week prior will incur a cancellation fee of 20%. ●Cancellations between one week and 24 hours prior will incur a cancellation fee of 50%. ●Cancellations within 24 hours of the dive will incur a 100% cancellation fee. If conditions worsen between dives during a Certified dive tour, the cost of the cancelled dive will be refunded. If conditions call for cancelling all dives, the entire trip will be refunded. In some cases, the location of the dive site may be changed, based on the conditions. In the case of a typhoon, participants will be contacted the day before the booking and the reschedule will be free of charge. Payment East Dive accepts payment of credit card at the Online. Payments via bank transfer also accepted. (Online payments will incur a 5% processing fee. This charge cannot be refunded.) Payments must be made by the date specified on the invoice. Participants will receive their invoice by email after booking. If the payment is not received by the specified date, the booking will be cancelled before contact by email. The reservation is not complete until payment has been received. Payments cannot be accepted on the day of the dive. Please complete the payment within 3 days of making a reservation. If the diving schedule is within a week, the payment deadline is the day after the application date. Refund All refunds will be Processed by domestic bank transfer.Even if you are a tourist, the transfer destination is only a domestic bank. Online payments will incur a 5% processing fee. This charge cannot be refunded.Please make a payment after fully understanding the contents. Refund date is the last day of the current month if payment is made by bank transfer If payment is made by credit card, the last day of the month following the date of payment Rescheduling Please contact East Dive by email or telephone in the event you wish to reschedule. All pool and open water diving requires a one-person minimum. If a booking is made by a single participant, it may be possible in the schedule to pair you with another diver. |
